If you are planning on volunteering for the upcoming healthexpo, please fill out volunteer form and get it back in as soon as possible. See flyer for date of the expo. Please send your completed forms to Pamela Shaw as we have to submit them to Tara Underwood as a group. You can indicate on your application your preferred hours of work (1/2 day or whole day). You may send to my email or fax at 706-323-0560, please send with fax cover addressed to Pamela Shaw, FNP. Thanks for giving back to the community and raising awareness to who we are and what we do!
